General Information
- The form must be duly COMPLETED and SIGNED with the COMPANY STAMP being affixed accordingly.
- Incomplete form including one that does not follow the specific registration format will be REJECTED.
- Please attach all relevant documents as specified in the General Guidelines.
- Vendor must inform MIMOS within 24 hours should there be any changes to your company’s information specified in the submitted VENDOR REGISTRATION form.
