General Information

 

  1. The form must be duly COMPLETED and SIGNED with the COMPANY STAMPbeing affixed accordingly.
  2. Incomplete form including one that does not follow the specific registration format will be REJECTED.
  3. Please attach all relevant documents as specified in the General Guidelines.
  4.     Vendor must inform MIMOS within 24 hours should there be any changes to your company’s information specified in the submitted VENDOR REGISTRATION form.
